this one helps. You can dump either the structure of the tables, or their content, using the mysqldump command-line tool.
For example, to get a dump of the structure (create table instructions) of all tables of a database, you can use something like this :
code :
mysqldump --no-data --user=USER_NAME --password=PASSWORD --host=HOST DATABASE_NAME
mysqldump --no-data --user=USER_NAME --password=PASSWORD --host=HOST DATABASE_NAME > dump-file.sql
mysql --user=NEW_USER_NAME --password=NEW_PASSWORD --host=NEW_HOST NEW_DATABASE_NAME < dump-file.sql

seems to work fine When you're gathering the file information you're overwriting $imagename on every loop so it will be assigned to the last one. Try attaching it to the $files variable (hopefully this doesn't mess with the upload class you're using).
code :
foreach ($l as $i => $v)
{
    if (!array_key_exists($i, $files))
    $files[$i] = array();
    $files[$i][$k] = $v;
    $files[$i]['imagename'] = $_POST['keyword'][$i];
}
$sql = 'INSERT INTO cover (id, img, keyword, category_name, cat_id)
     VALUES ("", "'.$this_upload['image'].'", "'.$file['imagename'].'",
         "'.$category_name.'", "'.$category.'")';

may help you . The way you get jQueryFileUploader to do what you want is to subclass the class that does all the work on the server.
If you look at the file structure on the server where you installed the FileUploader it looks something like this
code :
?php
   error_reporting(E_ALL | E_STRICT);
   require('UploadHandler.php');
   $upload_handler = new UploadHandler();
error_reporting(E_ALL | E_STRICT);
require('UploadHandler.php');

class MyUploadHandler extends UploadHandler
{
    /*
     * Maintain our database:
     * For each uploaded file, add an entry to the tbl_xxx
     */
    protected function handle_file_upload($uploaded_file, $name, $size,
                                          $type, $error, $index = null,
                                          $content_range = null)
    {
        // this does what woudl have automatically been done
        // before we subclasses anything
        $file = parent::handle_file_upload($uploaded_file, $name, $size,
                                           $type, $error, $index,
                                           $content_range);

        /* Now we add some extra processing
           if the file uploaded with no problems
         */
        if ( empty($file->error) ) {

            $sql = "INSERT INTO tbl_xxx ( a,b, imgpath)
                    VALUES('x', 'y', '$file->name' )";

            $db->query($sql);

        }
        return $file;
    }
} // end of class

$upload_handler = new MyUploadHandler($site_params);

Does that help The datetime literals needs to be in quotes like this: '2015-08-01 08:15:00' and you're also have a column name wrong in the table def.: (SUPPLY QTYinstead of SUPPLY_QTY).
This works:
code :
CREATE TABLE IF NOT EXISTS `Purchase` (
  `P_ORDERNO` int(5) NOT NULL auto_increment PRIMARY KEY,
  `P_ORDER_DATE` timestamp NULL,
  `P_ORDER_AMT` int NOT NULL,
  `SUPPLY_CODE` int(5) NOT NULL,
  `SUPPLY_DESC` varchar(50) NULL,
  `SUPPLY_QTY` int(5) NOT NULL
);

INSERT INTO `Purchase` (`P_ORDERNO`, `P_ORDER_DATE`, `P_ORDER_AMT`, `SUPPLY_CODE`, `SUPPLY_DESC`, `SUPPLY_QTY`) VALUES
(1, '2015-08-01 08:15:00', 30, 999, 'Hard drive', 300),
(2, '2015-08-02 08:15:00', 45, 999, 'Mouse', 310),
(3, '2015-08-03 08:15:00', 99, 784, 'Keyboard', 543);
